Just thought I'd post my interpretation of the various character profile layouts since a couple of people have been using it. Can be tweaked, polished, and customised to your heart's content. Enjoy!

Name: This is the name that your character was given at birth, irrespective of whether they are aware of/tell others what it actually is. For all intents and purposes, this is your character's real name - so for example "Betrand Davison".
Full Titles: Your character's official titles - not necessarily your in-game title. For nobility and those with Knighthoods, this might be something like Lord, Lady, Baron, Baroness, Duke, Duchess, Sir etc., but can extend to organisation positions such as Inquisitor, High Slayer and other similar ranks. They might also refer to noble deeds (as in the case of Demonslayer or Sorrowslayer), and just as frequently to less noble deeds (King/Queenslayer, Kinslayer etc.). Often, Lords and Ladies of houses will refer to their lands somewhere in these titles - "Lord Thomas Greenwood of Highdell" to give but one example of this.
Nickname/Alias: In a nutshell, this is what people actually call your character; it is what they are known as. This could be as simple as a shortened name (For example, a character called Christopher might be known as "Chris"), or else an alias - an alternate name or pseudonym - which might be a reflection of characteristics (think Curley, Crooks and Candy from "Of Mice and Men") or more like an adopted title such as "Swiftblade", blargtheawesome's character "Smoke" or Michcat's "the Physician".

[Picture] ...A picture, numbnuts. Preferably one that gives us the best idea of your character's appearance. Try not to go stealing things from sites like deviantart without the artist's prior permission however. Customise-y type things to "build" a character can be found on sites such as Doll Divine and (I hope I'm spelling this right-) Azaleas Dolls.

Age:
Quite simply, this is your character's age. Try to keep within the confines of the lifespan of their race, and if their age is measured in a different unit to years (for example the Soolerans use the number of Fadmas attended), try to post an equivalent in years for folks to lazy to read the Lore and find out for themselves.
Gender: It's not rocket science... Male or female, and if you can't tell you should have paid attention in biology. Note that some races are genderless.
Race: Here you should put your character's species, a list of which can be found in the Players' Handbook {here}. If your character belongs to a specific culture (which can be viewed on {this} page), that may be noted here too.
Social Status: The medieval times in Europe featured a feudal system ranging from peasants to royalty. This system is loosely divided into a majority of peasants ("villeins"), a large number of "freemen" (merchants, travellers, and the middle classes lucky enough to have decent lives), the knights, nobility and royalty. Of course, some nobility were more powerful than others, and in many cases religion held more power than the crown, but your character should fall into one of these categories anyway.
Sexuality: Fairly simple - if your character is attracted to members of the opposite sex they are heterosexual, the same sex; homosexual, or both; bisexual. If they are not attracted to either sex they are asexual.
Height: How tall is your character? Try to keep it realistic; we have far too many giants as it is, and people were shorter in the medieval times anyway. Again, keep within the confines of your race here - elves are slightly taller than humans while Dwarves and Halflings are quite a bit shorter. If possible try to include both metric and imperial measurements, else people tend to get confused. Can't find a converter? Here - let me Google that for you: {Click}.
Weight: Your character's weight. Same applies for this - think about their gender, age, and race before plucking a number from the sky, and try to include metric and imperial measurements.
